Applying for a Check

How to submit your own application for a check

You will need:

  • Information about your full legal name and any names you may have, including previous names, maiden name, names you have legally changed, aliases and informal names (like nicknames)
  • Your current address and 10 years of address history
  • If you are verifying your identity online a 100-point check is required

Please Note:

  • Once you begin your application you will receive an email from Account Security; the email will be called: Setup your account at forms.sa.gov.au.
  • This email will tell you to change your password which must be done before you start your application.
  • You must complete and submit your application within 28 days, or your application will be deleted and you will need to start the application again.

To complete your check:

  1. Go to https://www.dcsiscreening.sa.gov.au/SCRequestApplicationIndividual
  2. You will need to enter your name, date of birth, and email address. There is no need to add a reference number.
  3. Select Working with Children Check
  4. Under Application Type, select Volunteer (you will not be charged if volunteer option is selected)
  5. Complete the process, once cleared, submit the clearance to [email protected]
